- Was that address current. An address from an old session left open in a tab is a common cause and it looks like nothing is wrong.
- Did the full amount leave. Fee handling occasionally surprises people who set it manually.
Why a fresh address each time
One time destinations are how a wallet separates incoming payments without linking them, which is most of the point of the currency. Reusing an old one defeats that property and can credit the wrong place if it has been recycled on the platform side.
The version that is not about the deposit
If the payment went out from an exchange that knows who you are, straight to the deposit address, the deposit is probably fine and something else has happened that matters more. The chain hop is private and the exchange record is not, and a withdrawal followed minutes later by a matching deposit is a pattern nobody needed to read the chain to see.
